After immersing himself in college studies and honing his craft in off-Broadway theaters for over a decade, Chianese embarked on his formal acting journey by attending his inaugural professional class at HB Studio in Manhattan under the tutelage of esteemed instructor Walt Witcover. His Broadway debut came in 1965 with the production of Oliver!, marking the beginning of a career that would span Broadway, off-Broadway, and regional theater stages. During periods of struggle in securing acting roles, Chianese supplemented his income by showcasing his musical talents, strumming rhythm guitar and serenading patrons at taverns and eateries while also hosting open mic nights at Gerdes Folk City in Greenwich Village.
Chianese's breakthrough in television came at the recommendation of George C. Scott, landing him a role in the series East Side/West Side. His silver screen debut occurred in 1974 when filmmaker Francis Ford Coppola cast him as Johnny Ola in The Godfather Part II, setting the stage for a prolific film career that frequently saw him sharing screen time with Al Pacino in notable productions like Dog Day Afternoon (1975), ...And Justice For All (1979), and Looking for Richard (1996). Notably, Chianese also collaborated with Pacino in theatrical performances. Prior to his iconic role in The Godfather Part II, Chianese served as a recreational worker in rehab for the Drug Commission of New York, where he taught guitar to women incarcerated for drug offenses.


Renowned for his tenor vocal range, Chianese ventured into the world of music with the release of his album Hits in 2000, showcasing his talent through renditions of both American and Italian classics. His emotive performance of Salvatore Cardillo's "Core 'ngrato" ("Ungrateful Heart") on the season finale of The Sopranos garnered widespread acclaim. In 2003, in collaboration with AOL Music, he unveiled his second album, Ungrateful Heart, featuring a collection of 16 beloved Neapolitan songs. Throughout the 2000s, he delighted audiences with his mandolin prowess as part of a quartet's regular performances at the Hotel Edison.
Chianese made a memorable appearance as Leander Whitlock, a retired lawyer and influential figure, in the acclaimed HBO series Boardwalk Empire for three seasons.
